Forklift Pre-Operation Inspection Checklist

OSHA 1910.178(q)(7) requires a pre-shift inspection of every powered industrial truck. Sit-down counterbalance, stand-up, reach truck, and order-picker variants share this same checklist — fuel/power source captured per-truck via a configurable field.

WHAT IT IS

The Forklift Pre-Operation Inspection Checklist, explained.

The forklift pre-operation inspection checklist is the daily forklift inspection built to OSHA 1910.178. It runs 3 sections and roughly 23 pass / fail / N A checkpoints — covering Mast, Forks & Chains, Hydraulics & Controls, and Operator Compartment. Download the free printable sampleto put on a clipboard today — it's a basic quick-reference. The real power is running it in the DigiDocs app, where this becomes a fully customizable, 23-point digital inspection: every failure auto-creates a deficiency routed to your mechanic, photos attach on the spot, the operator e-signs, and each completed inspection becomes a signed, timestamped audit-trail record your auditor will accept without a fight.

CHECKLIST · STRUCTURE

What gets inspected.

This template is organized into 3 sections totaling roughly 23 Pass / Fail / N/A items. Clone it and tune fields to match your exact equipment configuration.

CREW · WHO RUNS THIS

Warehouse operators, distribution centers, manufacturing plants, retail-stocking crews, lumber yards, freight terminals, construction sites — any operation running Toyota, Yale, Hyster, Crown, Raymond, Mitsubishi, Komatsu, or Caterpillar forklifts. Every shift, every operator, every machine — OSHA's most consistent 1910.178(q)(7) finding is missed pre-shifts.

FIELD · INTEL

What inspectors catch most.

Pulled from competent-person write-ups in the field — not from a regulation digest. These are the items that fail the daily check more than any others.

  1. 01

Mast chain stretch over 2% — fork drop during full-height carry

  2. 02

Fork heel wear past 10% original thickness — capacity derating not applied

  3. 03

Seat belt missing or unsecured at one side — leading lateral-tipover injury cause

  4. 04

Propane tank seal weeping at the regulator inlet — daily often skips smell-test

  5. 05

Backup alarm disabled or volume reduced after operator complaints

OSHA · ENFORCEMENT

What a citation costs.

OSHA Serious: up to $16,131 per violation (2026 maxima). Willful or Repeat: up to $161,323. 1910.178 is the single most-cited general-industry OSHA standard year after year — typically appearing in the top three of the annual top-10 list. The pre-shift inspection requirement and operator certification are the two most-cited subsections.

THE PAPER SAMPLE VS · THE REAL THING

The PDF is the clipboard.
DigiDocs is the system.

A printable checklist still relies on someone remembering to do it, store it, and find it when an auditor or insurer asks. The same forklift pre-operationinspection in DigiDocs runs on the phone already in your operator's pocket — and turns a checkbox into a defensible compliance record the moment it's signed. Every field is yours to customize.

Every line item, not just sections

The full template carries all ~23 checkpoints with the OSHA / FMCSA / ASME citation on each — the sample only shows the section headers.

Photo proof on every failure

Operators attach a photo the instant something fails, so the defect is documented at the point of inspection — not reconstructed later.

Failures become mechanic work orders

Every Fail auto-creates a deficiency routed to your mechanic dashboard, with repair notes and auto-verify on the next inspection.

Red-tag / operability built in

Operator marks Operable / Non-Operable per OSHA 1926.1417; critical fails can lock the asset out of service automatically.

Signed, timestamped, permanent

Operator e-signature on submit; every completed inspection becomes an audit-trail PDF and a shareable, revocable customer link.

Make it yours

Rename fields, add sections, set conditional logic, schedule recurrences, and white-label it — no two fleets inspect exactly alike.

FREQUENTLY · ASKED

Forklift Pre-Operation, in practice.

Does every forklift need a daily inspection?

Yes. 1910.178(q)(7) covers every powered industrial truck — gas, diesel, electric, propane; counterbalance, reach truck, stand-up, order picker, walkie. The pre-shift applies to each one before each shift it's used. The inspection covers safety items, not maintenance; maintenance is on a separate schedule.

Who's qualified to do the forklift pre-shift?

The trained operator. 1910.178(l) operator training must include daily-inspection procedure, and the operator signs off on the inspection as part of pre-shift duties. A separate inspector is not required — but a supervisor reviewing the logs is the typical practice.

How long is the operator certification good for?

Three years per 1910.178(l)(4), with refresher training required immediately if the operator is observed unsafe, involved in an accident, gets a new truck type, or the workplace conditions change. The triennial re-evaluation has to be documented — OSHA asks for it during inspections.

What's the difference between OSHA's forklift rules in construction vs. general industry?

1910.178 (general industry) and 1926.602(c) (construction) cover similar ground. Construction sites running a forklift fall under 1926.602(c) but typically also apply 1910.178 because the operator training and daily-inspection requirements transfer directly.

What's the most common forklift citation?

Failure to ensure operator competency — usually because training records are missing or refresher training was overdue. The daily-inspection finding is second most common; 'no documented pre-shift' is the lead language OSHA uses.
