The pre-shift applies to each one before each shift it's used. The inspection covers safety items, not maintenance; maintenance is on a separate schedule.
The trained operator. 1910.178(l) operator training must include daily-inspection procedure, and the operator signs off on the inspection as part of pre-shift duties. A separate inspector is not required — but a supervisor reviewing the logs is the typical practice.
Three years per 1910.178(l)(4), with refresher training required immediately if the operator is observed unsafe, involved in an accident, gets a new truck type, or the workplace conditions change. The triennial re-evaluation has to be documented — OSHA asks for it during inspections.
1910.178 (general industry) and 1926.602(c) (construction) cover similar ground. Construction sites running a forklift fall under 1926.602(c) but typically also apply 1910.178 because the operator training and daily-inspection requirements transfer directly.
Failure to ensure operator competency — usually because training records are missing or refresher training was overdue. The daily-inspection finding is second most common; 'no documented pre-shift' is the lead language OSHA uses.
